Please complete this web form Cover Sheet for your Matching Grant Request. Before you submit the form, please use the printer icon at the top of the page to print at least one hard copy. You will need to send in a hard copy with the appropriate signatures and the necessary attachments. Once you complete the form and click "submit" FLH will be notified of your request. However, this does not complete your application. You will be redirected to the Grant Guidelines page, where you will find the list of required additional information. Additional information includes a budget, a letter of recommendation from your Branch Librarian, and the procedures for getting the necessary approvals and signatures. Once ALL information is received, the Affiliates Committee will review the grant request and will notify you of their decision within 60 days. The Affiliates Committee meets every other month.
Amount Requested is the amount you are asking FLH to fund. Please be aware that you CANNOT ask FLH for the full amount.
This is the amount of the match that your group will fund. (A matching grant does not mean that each funding source will put in the same amount.)
Please give as much detail as possible. You will also have to provide a budget with your completed application.
